what is it?
A bioidentical hormone has the exact molecular structure of hormones the body produces naturally. Bioidentical hormone replacement pellet therapy (BHRT) is a personalized approach to hormone replacement. It allows your body to absorb the right level of hormones at the right time using a delivery method the body can process efficiently.
Pellets are small capsules (about the size of a grain of rice) that contain bioidentical hormones like testosterone or estrogen. They are inserted under the skin, where hormones enter the bloodstream as they dissolve over several months.
what are pellets?
Hormones delivered by pellet therapy bypass the liver, do not affect clotting factors, do not increase the risk of thrombosis, and avoid the fluctuations of hormone levels some patients may experience using other methods. Biote bioidentical hormone pellets are made from soy or the yam plant to create estradiol and/or testosterone molecules, unlike synthetic hormones, which are made from animal parts or urine and have a different molecular structure than what the body produces.

The benefits of hormone replacement therapy may begin to work in up to four weeks as the bioidentical hormones enter the bloodstream. Some patients begin to feel the benefits of hormone therapy right away, while others won’t experience hormone therapy benefits for a few months. Many report feeling the full benefits after their second pellet insertion, although full hormone optimization can take up to six months.

Unlike other forms of hormone therapy, there are few side effects. Two percent of patients may experience mild acne or facial hair, and less than 1% experience hair thinning. All of these mild side effects are easily treatable. Because pellet therapy delivers consistent amounts of hormones 24 hours a day for 4 to 6 months, side effects are minimal when compared to other hormone delivery methods.
Pellet insertion is performed during a quick, 10-15 minute in-office procedure. Because they are inserted into the subcutaneous tissue (fatty layer) of the skin, the most common area for insertion is the upper buttocks or hip region.
Preparation: you will be asked to lie down on your side on the exam table. The area of insertion is identified, marked, and then thoroughly cleansed with alcohol. Local anesthesia is administered to numb the area and a small incision (approx. 1/4" long) is made into the skin.
Insertion: Using a special instrument, the provider inserts the hormone pellets through the incision and into the subcutaneous tissue. The incision is then closed with adhesive strips and a sterile dressing is applied to the area to promote healing and prevent infection.
Recovery:The patient is usually able to resume normal activities shortly after the procedure. However, it's important to avoid strenuous activity or keeping the insertion site clean and dry.
As each patient’s symptoms are unique, so is each patient’s path to hormone optimization. Some patients report some relief in as soon as four weeks, but full hormone optimization may take up to six months. Many report feeling more benefits after the first insertion and even better after the second.

For women, hormone pellets typically last approximately 3-4 months and for men, about 4-6 months. Although factors such as metabolism and dosing can play a role in duration, it is extremely important to regularly monitor your hormone levels so your provider can assess your response to the therapy and adjust the dosing and/or timing of pellet insertions to maintain optimal hormone levels and optimal benefits.
